Applications can be picked up and dropped off at the Chairman's Office, Tribal Health CEO Office at Eagle Butte Hospital - Cheyenne River Indian Health Service. Tribal Health Clinics and Testing Stations or by email at fieldhealth1@gmail.com
The Incentive for being Fully Vaccinated (2 shots) is still available.
